The patient is a 69-year-old female with a history of overweight BMI, CAD, HTN, hyperlipidemia, and aortic stenosis presenting with intermittent, less than 5-minute episodes of diffuse chest pain that began 5 days ago. The pain is worsened by mild exertion, relieved by rest within 3–5 minutes, and associated with dyspnea and fatigue during exertion. She denies recent URI, fever, weight loss, nausea, vomiting, or tobacco, alcohol, or drug use. Vitals reveal mild tachycardia, and physical exam identifies a pre-existing Grade II systolic murmur without other abnormalities.
